How an error would be verified, fixed and recorded — and the ledger of corrections made.

The policy

Every figure on this site must match the source's published figure for that hospital, surgery and period. When one does not, that is an error, it gets corrected, and the correction is recorded on this page — what was wrong, where, and when it was fixed.

How a report is checked

  1. Trace the figure to the pinned snapshot. The site is built from exact copies of each source release, pinned to SHA-256 checksums published on the data page — so what the build read can be re-examined byte for byte, by anyone.
  2. Compare. If the page disagrees with the snapshot, the fault is this site's — a transcription, derivation or labelling error. It is fixed, the site is rebuilt, and the correction is noted below.
  3. If the source itself has revised a figure, the new release is adopted through the normal data refresh, and the change shows up as a new reference period or release date on every affected figure — a data update, not a correction, because the site correctly showed what was published at the time.

Errors the machinery is built to prevent — a suppressed cell shown as a number, an old figure carried forward as current, a hospital ranked on data it did not publish — are treated as corrections too if one ever gets through. The methodology describes those rules.
